Position: Pharmacist, Staff PRN

Overview

Pharmacist, Staff PRN role at Comm Unity Care  Health Centers. This PRN pharmacist position involves collaborating with patients, families (as defined by the patient) and staff across all disciplines to provide pharmaceutical care services.

Responsibilities
  • Ensure that prescriptions are dispensed safely and accurately as prescribed.
  • Conduct patient drug regimen review.
  • Provide patient prescription counseling services as required.
  • Prepare and maintain required records as per requirements.
  • Communicate with patients, providers and other clinical staff as needed.
  • Perform other duties as assigned.
Primary Accountabilities
  • Ensure all tasks related to patient care, patient administrative processes and related duties comply with regulatory and accreditation standards, including The Joint Commission and Comm Unity Care  SOPs and policies.
  • Develop and maintain favorable internal relationships and partnerships with co-workers.
  • Interact respectfully and collaboratively with patients and their families to foster positive relationships.
  • Collaborate with all care team members to provide patient-centered care.
  • Ensure all actions, job performance, conduct and communications represent Comm Unity Care  professionally.
  • Uphold compliance with company policies and the organization’s mission and values.
  • Complete all mandatory training and maintain appropriate credentials/licensure.
Essential Functions / Key Competencies
  • Strong customer service and relationship-building skills.
  • In-depth knowledge of drug therapy and dispensing medications.
  • Basic computer skills, including Microsoft Office.
  • High attention to detail and accuracy.
  • Excellent verbal and written communication skills.
Education
  • Bachelor’s degree in Pharmacy or Doctor of Pharmacy (Pharm

    D) required.
Experience
  • 5-7 years of experience in retail (community) or mail order pharmacy required.
Required Certifications / Licensure
  • Current license by Texas State Board of Pharmacy required.
  • Current Health Care Provider CPR certification (American Heart Association or American Red Cross) required and must be maintained during employment.
