Job Description Summary

The Traveling physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Assignment Details
  • Duration: 13 weeks (standard travel assignments) unless otherwise agreed upon.
  • Weekly hours:
    Full-time schedule of 40 hours per week.
  • Starting location:
    Santa Cruz.
  • Work schedule will vary based on assignment location; weekend availability may be required depending on patient needs.
  • Employment status:
    Direct employee of Bridge Home Health & Wellness. May be reassigned to new contracts within the State of California after completion of an assignment.

Must adhere to company attendance and reliability policies to deliver high-quality and consistent patient care.

Training & Team Support

Willingness to assist in onboarding and training new employees as requested and serve as a thought leader by sharing knowledge and best practices with the team.

Responsibilities
  • Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
  • Assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care.
  • Assess muscle strength, mobility, gait, ROM, and potential for rehab.
  • Direct physical therapy treatment and instruct patients and families/caregivers on the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy.
  • Report to physician patient’s reaction to treatment or changes in condition.
  • Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy.
  • Train patients in the use of prosthetic devices if required.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Prepare and submit clinical and progress summaries based on goal attainment.
  • Provide physical therapy consultation to home families/caregivers when indicated.
  • Provide in-service education programs for nursing organization personnel as needed.
  • Participate in peer consultation process.
  • Supervise physical therapy assistants according to organization policy and state regulations.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
  • Must adhere to the company attendance and reliability guidelines.
Qualifications
  • Post baccalaureate degree in physical therapy (Doctor of Physical Therapy) approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
  • For physical therapists licensed prior to January 2016 change, may possess a baccalaureate or master’s in physical therapy from an approved program.
  • Licensed to practice as a physical therapist by the Physical Therapy Board of California.
  • Good verbal and written communication and organization skills.
  • Current CPR certification.
  • Valid auto insurance and driver's license with a vehicle in good working order and insured as required.
  • Must reside 55 miles or more from work location.
  • Must have reliable transportation.
