Role Description :

Provides direct patient care, evaluates functional needs and outcomes, and collaborates with other providers in the clinic. Reviews patients functional needs and adjusts care plans to ensure optimal patient outcomes.

Administers physical therapy treatments such as patient exercise using equipment and objective functional testing.

Employs treatment strategies that utilize proven treatment strategies aimed at achieving patient outcomes and patient retention.

Conducts therapy practice within defined standards of care while maintaining all ethical, legal and professional responsibilities.

Directs the assistance of support staff to achieve patients functional goals.

Communicate effectively with DOR, teammates and facility staff.

Maintains excellent documentation with evaluations, treatment goals, and plans. Regularly updates documentation with confidentiality while maintaining all prescriptions and signatures.
