Position: Nurse Practitioner (Sleep/Pulmonary)
The Nurse Practitioner (NP) SLEEP/PULMONARY practices in the Pulmonary, Critical Care, and Sleep section of the Medicine Service to provide a full range of pulmonary and sleep services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission.

The Sleep/Pulmonary NP for the Pulmonary and Critical Care section will primarily be involved with patient care and administrative needs for the sleep program within the Eastern Colorado Healthcare System or outlier Healthcare Systems that send Interfacility consults (IFCs) to our site for review or assistance. While the majority of work will be outpatient based clinics, there may be occasional inpatient sleep consultations requiring evaluation and care as well.

Additional roles include, but are not limited to, assisting the Referral Coordination Initiative nurses with Sleep and/or Pulmonary Consult Triage, completion of EConsult requests, review of patient PAP use/adherence reports, and completion of required Department of Transportation, Federal Aviation Administration, or other patient paperwork confirming adherence to sleep apnea treatment plans.

Duties and responsibilities include, but are not limited to, the following:

* Independently manages a panel of patients with acute and chronic health conditions

with an emphasis on health promotion and disease prevention using evidence-based

practice.

* Develops and implements an individualized plan to achieve optimal population health

and disease management.

* Demonstrates leadership and works collaboratively with the interdisciplinary team.

* Orders and interprets labs, tests, and diagnostic studies.

* Prescribes pharmacologic and non-pharmacologic interventions.

* Initiates referrals and consults to specialty services.

* Provides education to patients and their families.

* Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.

Pay:
