Description

Children's Hospital Lifestyle Medicine Tier 2 (LM2) Program provides comprehensive health care integrated with research study participation to children, adolescents and young adults with obesity and obesity-related comorbidities, including type 2 diabetes (T2D), polycystic ovary syndrome (PCOS), hypertension, metabolic-associated steatotic liver disease, dyslipidemia, sleep apnea, and more. The main focus of this position is to provide initial evaluation and care of patients with newly diagnosed T2D as well as new and follow‑up care in our multidisciplinary LM2 clinic.

In this role you will work closely with an interdisciplinary team of physicians, nurse practitioners, certified diabetes educators, nurses, dietitians, psychologists, social workers, and pharmacists to improve health outcomes for youth with obesity and related comorbidities.

Key Responsibilities
  • Triage referrals for pre‑adolescents/adolescents/young adults (ages 8‑21) in the LM2 EPIC pool, coordinating with LM1 and on‑call providers to schedule initial care and address lost‑to‑follow‑up, hospitalized, or acutely decompensated patients.
  • Perform detailed history, physical examination, and formulate differential diagnosis within scope of practice for clients with new‑onset diabetes, including education on medication administration, injections, and self‑monitoring.
  • Coordinate follow‑up care and clinical visits in a multidisciplinary clinic for patients with T2D, including medical treatment, bariatric surgery referrals, and screening for associated comorbidities.
  • Provide clinical visits for insulin‑resistance–associated conditions such as hypertriglyceridemia and PCOS, managing primary conditions and associated comorbidities.
  • Assess home blood glucose monitoring, continuous glucose monitoring, and medication dosing between visits to adjust regimens.
  • Manage outpatient follow‑up for babies with stress hyperinsulinism, secondary adrenal insufficiency, and other endocrine emergencies, including medication titration and scheduling of confirmatory testing.
  • Establish systematic follow‑up to evaluate and modify the plan of care, including referrals and consultations as appropriate.
  • Ensure thorough patient and family teaching to manage treatment plans at home and promote self‑care.
  • Collaborate with program CDE/RN to standardize educational materials, streamline clinical processes, and participate in quality improvement initiatives.
  • Work onsite in Aurora, CO.
Minimum Qualifications
  • Master's degree in nursing;
    Registered Nurse license and certification as a Family Practice or Pediatric Nurse Practitioner; or Physician Assistant (PA) degree.
  • Valid license to practice as a nurse practitioner or physician assistant in the State of Colorado.
  • One year of direct patient care experience.
  • Current drug test clearance through Children’s Hospital Colorado.
  • Successful completion of a national criminal background check.
Preferred Qualifications
  • Professional background in pediatrics.
  • Professional background in diabetes management.
  • Experience working with the Latino community and other marginalized populations.
  • Experience in obesity and/or adolescent care.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Basic Life Support (BLS) certification.
  • Spanish language proficiency.
  • Prescription authority (DEA).
  • Advanced practitioner knowledge, pediatric diagnostics skills, and proficiency with electronic medical records.
  • Excellent communication skills and professionalism.
  • Ability to work well with multidisciplinary team members.
