• Assist in maintaining current files, treatment records, resident files, and progress notes.
• Review request form for restorative care and physicians’ orders.
• Prepare resident for treatment by dress/position and administer restorative care.
• Assist with instructing and supervising resident during therapy.
• Inform the resident of consequences of not participating in prescribed therapy.
• Participate in discharge planning, care plans, and resident assessments.
• Report changes in resident status to supervisor.
• Responsible for cleaning common areas and apartments.
• Perform all other duties as assigned.

• High School Diploma or GED and certification from an accredited nursing program.
• Certified Nursing Assistant (CNA) License for the state of Colorado or ability to convert within 30 days.
• PCC (Point Click Care) experience is a plus.
• Current CPR/BLS from American Heart Association or American Red Cross.
• Current proof of a Flu Vaccination within the last 12 months.
• COVID VACCINE OR QUALIFYING EXEMPTION REQUIRED.
• Ability to pass a criminal background check and Colorado CAPS background check.
