Job Summary:
The Nurse Navigator - Neurosurgery serves as a clinical expert and patient advocate, guiding individuals and their families through the complex continuum of neurosurgical care. This role coordinates care from initial referral through diagnosis, surgical intervention, recovery, and follow-up, ensuring seamless communication among patients, caregivers, and the multidisciplinary neurosurgical team. The Nurse Navigator promotes high-quality, patient-centered care by reducing barriers, improving outcomes, and enhancing the overall patient experience within the neurosurgery service line.
Key Responsibilities:
+ Serve as the primary point of contact for neurosurgery patients, providing education, emotional support, and care coordination throughout the treatment journey.
+ Collaborate closely with neurosurgeons, advanced practice providers, nursing staff, rehabilitation services, and ancillary departments to ensure timely and efficient care delivery.
+ Coordinate diagnostic testing, consultations, surgical scheduling, inpatient transitions, and post-operative follow-up care.
+ Assess patient needs, identify barriers to care, and facilitate access to appropriate clinical, psychosocial, and community resources.
+ Provide comprehensive patient and family education related to neurosurgical conditions, procedures, treatment options, recovery expectations, and symptom management.
+ Monitor patient progress and outcomes, proactively addressing complications, concerns, or changes in clinical status.
+ Support informed decision-making by ensuring patients understand their care plans and available treatment options.
+ Promote continuity of care across inpatient, outpatient, and post-acute settings, including coordination with rehabilitation and home care services.
+ Participate in quality improvement initiatives, data collection, and performance metrics related to neurosurgical patient outcomes and satisfaction.
+ Advocate for patients while upholding professional nursing standards, regulatory requirements, and evidence-based practice.
Requirements:
+ Minimum Required
Education:
Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years experience. Preferred:
Bachelor's degree in Nursing.
+ Required
Licensure/Certification :
State licensure as a Registered Nurse (RN). Relevant life support certification as determined at position level.
Minimum Experience:
2 years of nursing experience.
+ BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire.
