Registered Nurse (Community Nursing Home Coordinator)
Pay Range: $88,444.00 - $ per year.
Base pay range: $88,444.00/yr – $/yr.
Summary
The Registered Nurse will assume responsibility for the coordination of care focused on education, self‑management, and customer satisfaction throughout the continuum of care while following procedures per established policies and guidelines.
- English language proficiency required.
- Graduate of a school of professional nursing approved by an accrediting agency (ACEN or CCNE).
- Graduates of foreign schools may provide statutory equivalent qualifications as described.
- State licensing required; temporary appointment possible while completing bridge program.
- Preferred experience:
geriatric and nursing home experience; must hold a valid driver’s license and be able to drive a Government Official Vehicle. - Physical requirements:

- Coordinate care and discharge planning across inpatient and outpatient settings.
- Assist veterans, families and caregivers with services for complex health care needs.
- Manage and coordinate transitions to community nursing homes, ensuring continuity of care.
- Perform clinical nursing reviews of medical records for non‑VA hospitals to determine stability and transfer potential.
- Advocate holistic care through patient and family education and provider interaction.
- Serve as Contract Office Representative and Ordering Officer, renewing community nursing home contracts.
- Conduct follow‑up visits to veterans post‑admission to CNH to ensure safe, quality care.
- Work closely with contracting office to pursue contracts that enhance veterans' quality of life.
