2nd Home Skilled Service is an in home health provider. We need Nurses for Part Time in the following service areas:
Denver, Aurora, Littleton, Englewood, Lakewood, Highlands Ranch, Commerce City, Arvada, Westminster.

• One to one patient care in a home environment
• Execution of physicians prescribed plan of care and compliant documentation of care
• Administration of prescribed medication, treatments and therapies
• Patient assessments, care planning (SOC and follow-up visits) and coordination of care with physicians and other disciplines
• Provide effective communication to patient/family and team
• Work independently and use electronic documentation systems

• Required:
  
  RN License
• Home & community care experience
• Knowledge/skills with short skilled nursing visits
• Must be willing to provide in home nursing care
• Current CPR certification
• Experience with electronic documentation systems
• Document management systems (preferred)
• RN experience (2-5 years) (preferred)
