Clinical Therapist II – Fitzsimons Veterans Community Living Center – Aurora
Colorado Department of Human Services is hiring a full‑time Clinical Therapist II to provide person‑centered care at the Fitzsimons Veterans Community Living Center, a 180‑bed skilled nursing facility serving veterans.
Shift Monday through Saturday or Sunday through Thursday (subject to change). You will work at the assigned location during scheduled hours and may cover weekends, evenings, and holidays as needed.
Job Purpose to enable residents to enhance their quality of life through leisure and recreational activities, supporting physical, cognitive, social, and spiritual well‑being.
Key Responsibilities- Plan, facilitate, and evaluate a diversified recreation and leisure program for residents.
- Develop a program calendar in collaboration with staff.
- Ensure programming includes cognitive stimulation, cultural, social, educational, recreational, physical, spiritual, psychological, and creative activities.
- Provide one‑to‑one visits with residents per care plan.
- Complete daily documentation of resident participation.
- Conduct and assist with resident outings.
- Complete assessments, quarterly/annual reviews, care plans, and MDS documentation.
- Coordinate services with dietary and housekeeping, and secure supplies for programs.
- Perform other duties as assigned.
- Five (5) years of experience working with seniors/memory care or in a long‑term care setting.
- Or a bachelor’s degree in Recreation, Social Work, Art Therapy, Music Therapy, or a closely related field PLUS one year of relevant experience.
- Experience in memory‑care, long‑term care, or assisted‑living environments.
- Experience facilitating or assisting with programming for seniors.
- Experience in art or music therapy.
- Strong documentation, charting, and writing skills.
- Experience with electronic medical records.
- Full background check, including CBI & FBI fingerprint, Colorado Court Database, Medicare Fraud Database, TRAILS Database, and CAPS check where applicable.
- Must be a Colorado resident at time of application and maintain residency throughout employment.
- Must possess valid, non‑restricted Colorado driver’s license or obtain one within 30 days if holding out‑of‑state license.
- Must pass TB and required vaccinations.
