We are seeking a caring and dynamic Occupational Therapist to work in a skilled nursing environment, helping patients regain a quality of life. If you have a passion for providing seniors with short- and long-term care and meet our qualifications, we want to speak with you.

JOB SUMMARY

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following regulatory and clinical practice requirements. This includes recovery, improvement in function, and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

JOB DUTIES
  • Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient’s progress and adjusts treatments accordingly.
  • Evaluates treatment outcomes for effectiveness; modifies services and makes referrals based on outcomes.
  • Provides consultation and counseling to patients, families, caregivers and other providers.
  • Communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary per regulatory requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system.
  • Accurately records treatment minutes for services provided.
  • Participates in required meetings: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Participates in facility and company required trainings, in‑services and conferences.
  • Able to supervise and support PT assistants, rehabilitation aides and/or students.
  • Adheres to NBCOT /AOTA and State practice acts, code of conduct, ethics.
  • Adheres to company policies, procedures and code of conduct.
  • Maintains active State licensure.
REQUIREMENTS & EXPERIENCE
  • Bachelor’s degree or higher in Occupational Therapy or related field.
  • Active state license.
  • 2+ years of experience in a skilled nursing facility environment.
WORK SCHEDULE & PAY RATES

Full-Time / Part-Time / PRN. Pay: $45.00 – $55.00 per hour, depending on experience.
