Case Manager - Early Psychosis Intervention Program

We are hiring three (3) Case Managers on our Early Psychosis Intervention Program.

Positions
  • Two (2) Permanent full‑time positions, 35 hours per week, evenings & weekends may be required.
  • One (1) 1‑Year Contract full‑time position, 35 hours per week, evenings & weekends may be required.
Pay Range

CA $56,444.00/yr – CA $68,429.00/yr

Location

The successful candidates will work primarily from our Aurora Office (15150 Yonge Street, Unit 201, Aurora ON L4G 1M2) and out in the community for client visits and program needs, with regular travel in York Region & South Simcoe required.

Job Profile

The Case Manager works with clients 14 to 35 years of age who are experiencing or in the early stages of their first episode of psychosis. The Case Manager collaborates with clients and the team to assess, plan, coordinate and provide treatment, rehabilitation and support. The purpose of early intervention is to utilize and support bio‑psychosocial interventions, using natural supports to the greatest extent possible, to enable the client to maintain and develop age‑appropriate functioning and roles.

Responsibilities
  • Provide individualized and flexible services to engage, support, and integrate clients into the community.
  • Facilitate use of resources and supports to maximize client independence and potential.
  • Adhere to agency Health & Safety Program and Client Safety Framework.
  • Participate in individual and systemic advocacy initiatives to ensure clients’ wishes are heard and addressed.
  • Collaborate with interdisciplinary teams, maintain accurate documentation, and attend relevant meetings.
Requirements
  • A degree or diploma in the health or social services; a related discipline is preferred.
  • At least one year of experience in a community‑based organization working with adults with serious mental illness.
  • Current registration with OCSWSSW or a regulatory body under the Regulated Health Professions Act.
  • A second language reflecting the local community is an asset.
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act, and PHIPPA requirements.
  • Knowledge of Recovery principles and empowerment‑oriented practices.
  • Extensive knowledge of supports and services in York Region and South Simcoe.
  • Understanding of systemic issues such as poverty, unemployment, stigma, and isolation.
  • Ability to observe boundaries, facilitate emotional regulation, avoid dual relationships, maintain confidentiality, and engage in self‑care.
  • Excellent oral and written communication skills.
  • Computer proficiency, particularly Microsoft Office and Outlook.
  • Valid Canadian Full G driver’s licence; minimum $1,000,000 third‑party liability insurance covering use of a personal vehicle for client transport.
  • Current Vulnerable Sector Screening (Police Check).
