Job Summary

The Service Coordinator is responsible for basic case management and day‑to‑day coordination of supportive services activities performed on behalf of the residents of the property, with emphasis being placed on those who are frail and se services will include, but are not limited to arranging for personal assistance, homemaking, meals‑on‑wheels, transportation, preventative health screening and advocacy.

Essential Duties & Responsibilities
  • Establish and maintain relationships with local, state, and federal agencies and local service providers, such as the Area Agency on Aging and its subcontractors.
  • Compare costs of supportive services to determine the “best deals” in pricing to ensure individualized, flexible, and creative service provision.
  • Develop and maintain a current directory of community‑based providers for use by both project staff and residents.
  • Read and be familiar with the resident lease and property house rules, and inform the Community Manager of any noted lease violation.
  • Identify at‑risk and frail residents who most require support services.
  • Basic case management of individual residents, including assessment of individual needs for service, identification of services to be arranged, arrangement of appropriate services, monitoring the quality of services provided, ongoing reassessment of resident need, follow‑up of ongoing service provision, and disposition/termination of services.
  • Maintain a current file on each resident client, documenting all steps in basic case management, including progress notes and case resolution.
  • Arrange wellness/preventive medicine screenings, clinics, and speakers on all medical and legal issues of importance to the elderly.
  • Help residents build informal support networks with other residents, family, and friends.
  • Assist residents and families in obtaining necessary services and support decisions regarding residency or moving to a higher level of care.
  • Coordinate all aspects of the Service Coordinator program with the Community Manager on an ongoing basis.
  • Submit a weekly report of all pertinent activities to the Regional Service Coordinator for Service Programs.
  • Perform other duties as assigned.
Education & Experience
  • A bachelor’s degree in social work, gerontology, or other social science is preferred; however an equivalent of five years of relevant work experience may be considered in lieu of a college degree.
  • Evidence of prior networking ability is required, as is the ability to relate well with senior citizens.
  • Experience determining and arranging services for the elderly is essential.
