We seeking to partner with a dedicated and compassionate Nurse Practitioner to deliver advanced in-home wound care for chronic and complex wounds. The ideal candidate will provide high-quality patient care, perform assessments, and develop treatment plans in the patient's home or potentially in your office. This role requires a strong commitment to patient advocacy and the ability to work collaboratively with other healthcare professionals.

• Conduct comprehensive wound examinations and health assessments for patients upon referral.
• Debride and graft chronic wounds that have not healed with at least 4 weeks of conservative treatment.
• Collaborate with physicians and other healthcare providers to ensure coordinated care.
• Maintain accurate medical records and documentation in compliance with healthcare regulations.

• Independent Medicare Part B credentials are strongly preferred.
• Experience in wound care is highly desirable.
• Ability to manage medical records efficiently while ensuring patient confidentiality.
• Excellent communication skills for effective patient interaction and collaboration with healthcare teams.
• Strong organizational skills to manage multiple patient cases simultaneously.

• Already Credentialed with Medicare & Advantage Plans
• LLC already created
• Ability to work mobile (Treat Patient in-Home)
• Wound Care and Advanced Wound Care experience
