Job SummaryThe Pharmacist is responsible for appropriate, evidence-based medication use for the institution. They ensure patients get the right medication at the right dose. Pharmacists must have a medication safety viewpoint and be in compliance with all applicable standards and regulations (TJC, BOP, CMS, etc). Pharmacists oversee the activities of pharmacy technicians and engage in teaching as part of the hospital's mission.
CoreResponsibilities and Essential Functions
- Provides clinical pharmaceutical care to patients, preventing, detecting, and resolving medication-related problems and making appropriate evidence-based, patient-centered medication therapy recommendations.
- Participates as an effective member of interdisciplinary care teams, communicating follow-up with the team.
- Effectively verifies patient medication orders for appropriateness and accuracy.
- Provides effective medication education to patients, families, and team.
- Actively participates in the development and implementation of performance/quality improvement and patient safety initiatives, including medication event/adverse reaction reporting and follow-up, clinical guideline development, disease management, medication use evaluation/improvement, and cost containment actions.
- Represents pharmacy services on institutional/practice area committees and quality unit councils.
- Conducts monthly unit inspections of assigned area.
- Demonstrates proficiency in pharmacy distributive services, ensuring quality, safety, and efficiency in the distribution process.
- Collaborates and demonstrates teamwork with pharmacy coworkers to ensure communication, excellent customer service, and safe and effective drug distribution.
- Provides oversight for pharmacy technicians and participates in opportunities to improve the quality, effectiveness, and/or efficiency of medication distribution.
- Supports the education and professional development of the Pharmacy Department and other hospital disciplines by precepting residents, pharmacy students, or pharmacy technicians as assigned.
- Engages in professional self-development, completing medical center/pharmacy competency requirements by deadlines and attending seminars, continuing education programs, department meetings, etc.
- Demonstrates active membership, service, and leadership in professional organizations.
- Complies with all Wellstar Health System policies, standards of work, and code of conduct.
Minimum Education
- Bachelors in Pharmacy
- Doctorate in Pharmacy (Preferred)
- Registered Pharmacist
- Minimum 2 years in Pharmacy as a pharmacy technician, intern, or pharmacist (Preferred)
