FT Starting salary $60,000.00
PT Starting salary $36.00 per hour.

Georgia Department of Public Health Commissioner and leaders encourages all employees to engage in regular wellness activities and to make lifestyle choices that promote health and well‑being. Job Responsibilities- Under broad supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures.
- Focuses on prevention of illness, injury, or disability, and promotion of health and maintenance of the health of populations.
- Cares for populations with special needs during disasters/emergencies and biological and chemical protocols.
- Identifies high risk populations and tailors interventions based on the needs of the population and resources available.
- Provides home‑visiting services to identify, assess and provide care coordination services to eligible perinatal patients and qualifying infants.
- Coordinates program services with other service providers and community organizations to promote optimal implementation.
- Screens and refers clients to other community service providers.
- Monitors per program guidelines, providing educational instruction and materials to promote wellness, safety, and pregnancy and postpartum complications.
- Monitors infant complications including feeding, weight, head circumference, and developmental screening.
- Develops a plan of care for individuals and families, modifying treatment plans as indicated by client’s responses and conditions.
- Provides referral/linkage when medical and environmental risks are identified to appropriate services and promotes the program through community engagement and outreach events.
- Develops and sustains community partnerships.
- Provides home health visits in Richmond, Emanuel, Jenkins, Burke, Screven, and Jefferson County.
Bachelor’s degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification, OR associate degree in nursing from an accredited college or university AND one year of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certification.
