An FQHC in Augusta, GA is seeking an experienced Nurse Practitioner or Physician Assistant to join our established outpatient practice. This role is ideal for a provider who is passionate about chronic disease management and family medicine and values long-term patient relationships.
Responsibilities- Provide high-quality primary care services in an outpatient setting
- Deliver comprehensive care focused on chronic disease management and family medicine
- Collaborate with a multidisciplinary team to coordinate patient care
- Minimum of 5 years of clinical experience required (new graduates cannot be considered)
- Strong interest and experience in chronic care management and family medicine
- Full-time position
