Key Responsibilities:

• Assess resident health conditions and contribute to the development of care plans.
• Administer medications and treatments as prescribed.
• Monitor residents' progress and respond to changes in their condition.
• Provide education and health coaching to residents and their families regarding health management.
• Communicate effectively with residents, families, and interdisciplinary teams.
• Ensure compliance with all safety, infection control, and regulatory guidelines.
• Ensures that a safe and effective resident care environment is maintained.
• Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team.
• Safely administers and documents all forms of medication administration.
• Assesses, plans, implements and evaluates care based on age-specific components.
• Identify and assess resource utilization and safety issues, taking appropriate action.
• Aligns nursing/clinical objectives with organizational strategic outcomes, participating in strategy planning and execution.
• Treats all residents, visitors, and staff with courtesy and dignity.

Qualifications:

• Current and valid licensure as a Registered Nurse in the State of New York.
• Ability to work effectively within a multidisciplinary team.
