Summary
Provides professional patient care and practices physical therapy by implementing and documenting treatment following evaluation by the physical therapist. Treats patients in accordance with the order from the physician and the plan of care established by the physical therapist and provides treatment through the use of a variety of therapeutic procedures and assistive devices intended to maximize functional potential in all areas of the patient's life.
Implements an individualized treatment plan based on the therapist's evaluation and plan of care to address patient needs. Collaborates with Physical Therapist to identify updates and revisions to treatment plans, as needed, throughout the treatment process. Establishes and instructs patients, families, and other caregivers in exercises and therapeutic home programs to be continued following discharge. Completes the appropriate documentation and billing in a timely manner.
All care is provided in accordance with the philosophy and essential functions of the hospital, the rehabilitation department, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.
Must have an Associate’s or Bachelor's degree from an accredited PTA program.
Licensure/CertificationMust hold or be eligible to receive a Physical Therapist Assistant license in the state where services will be provided within the first 90 days of employment. Must be CPR-certified or must become certified within the first 90 days of employment. Must maintain all certifications and license according to the expiration date. American Physical Therapy Association membership preferred. If assigned to drive a company vehicle, a valid driver's license is required.
If assigned to drive a personal vehicle, proof of insurance is required in addition to a valid driver's license.
Work experience preferred, but not required, other than clinical experience within educational training program.
Other QualificationsMust be able to lift 50 pounds, assist with 100 pounds, may be required to push up to 300 pounds. Must have good verbal and written communication skills. Must be willing to work at other locations as assigned.
