LPN - PRIMARY CARE QUALITY PROGRAMS EAMC MISSION

POSITION SUMMARYLPN will collaborate with quality team and primary care providers to improve: risk diagnosis coding, medication reconciliations, statin measures, disease specific patient education, and transitions of care.
POSITION QUALIFICATIONS- Minimum Education
- Graduation from an accredited LPN program
- Minimum Experience
- 1 year hospital experience
- Required Registration/License/Certification
- BLS obtained within 90 days of hire or transfer
- Licensed Practical Nursing license
- Preferred Education
- N/A
- Preferred Experience
- 1+ years clinical experience
- Proficiency with Microsoft Excel
- Preferred Registration/License/Certification
