Working closely with a dedicated team of physicians and clinicians, you'll develop and manage care plans that support recovery and help patients get back to the life they love.

As a Home Health Registered Nurse, you will:

Provide admission, case management, and follow-up skilled nursing visits for home health patients.

Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.

Confer with physicians to develop initial treatment plans based on orders and assessments, providing hands-on care, management, evaluation, and patient education under supervision.

Revise care plans in consultation with physicians based on ongoing assessments and policy/regulation requirements.

Coordinate appropriate care with various healthcare personnel and external providers.

Communicate regularly with the patient's physician and Clinical Manager.

Implement care plans in conjunction with patients and families to achieve optimal health outcomes.

Coordinate, oversee, and supervise the work of Home Health Aides, including providing written care instructions and monitoring documentation completion.

Discharge patients after consultation with the physician and Clinical Manager, ensuring proper documentation.

Prepare medical documentation including case conferences, patient contacts, medication changes, re-certifications, and care plan updates.

Use your skills to make an impact

Diploma, Associate, or Bachelor Degree in Nursing.
Minimum of one year nursing experience preferred, with strong med-surg, ICU, ER, or acute care background.
Current and unrestricted RN licensure, CPR certification.
Strong organizational and communication skills.
Valid driver's license, auto insurance, and reliable transportation.

Pay Range: $77,600 - $106,600 per year.
