Overview

The Director, Nurse Practitioner Practice Excellence supports the strategic execution and educational growth of the Nurse Practitioner program across the organization. Reporting to the AVP, Nurse Practitioner Program, this position partners in driving provider excellence, documentation integrity, and performance improvement within a large, geographically dispersed NP workforce.

The Director, Nurse Practitioner Practice Excellence is responsible for contributing to the ongoing development and success of the Nurse Practitioner (NP) and the NP program through onboarding, orientation, training, continuous process improvements and professional development, along with other miscellaneous duties. This role collaborates closely with Clinical Affairs leaders, Compliance, Credentialing, and Operations to ensure alignment with regulatory standards and organizational benchmarks.

Functions
  • Serve as liaison within the Company between the Credentialing department, Compliance department, HR department, Clinical Affairs, and Operations.
  • Utilizes expertise as a subject matter expert to support the Hospice Nurse Practitioner Program through developing, redefining, and maintaining Hospice Nurse Practitioner materials for onboarding, orientation, continuing education, clinical best practices, etc.
  • Facilitate Welcome calls and initial onboarding communications with Operations and Hospice Nurse Practitioners to ensure seamless orientation.
  • Coordinate and facilitate orientation sessions for Hospice Nurse Practitioners.
  • Conduct and document 30/60/90-day check-ins with new hires, as well as additional touch points as appropriate, tracking onboarding milestones and identifying early support needs.
  • Ensure that databases of the employed Hospice Nurse Practitioner are complete and accurate including items related to Orientation and ongoing education.
  • Develop and implement quality care initiatives to enhance patient care and service delivery.
  • Monitor and analyze patient care metrics to identify areas for improvement.
  • Provides hospice-based education, orientation, and in-servicing as needed.
  • Supports Hospice Nurse Practitioners by being available to assist with questions or concerns including for clinical issues as needed.
  • Conduct biannual/annual check-ins with Hospice Nurse Practitioners.
  • Lead NP town halls and professional development forums, developing and delivering educational sessions focused on quality, safety, compliance, and patient outcomes pertinent to the Hospice Nurse Practitioner.
  • Collaborate with the team to support Ongoing Professional Practice Evaluations (OPPEs) for Hospice Nurse Practitioners, guiding individualized feedback and professional development.
  • Ensure performance data (documentation quality, prescribing trends, patient outcomes, visit metrics) are collected and reviewed consistently.
  • Performs occasional site visits to provide leadership, perform reviews, and offer support.
  • Assists with obtaining collaborative agreements between Physicians and Nurse Practitioners when appropriate and approved by legal, CMO, and the Director, Hospice Nurse Practitioner Program.
  • Represents the organization in local, regional, and national forums related to advanced nursing practice.
  • Ensures compliance with federal, state, and local regulation related to hospice care.
  • Other miscellaneous duties as directed by supervisor.
About You
  • Master’s or higher degree in Nursing (MSN or DNP) with completion of graduate-level nurse practitioner education program accredited by a national accrediting body.
  • Current state license as a registered nurse AND advanced practice nurse/nurse practitioner.
  • Current national certification as a Nurse Practitioner (ANCC or AANP).
  • Minimum of five years of experience as Nurse Practitioner, with three years in a hospice and palliative settings.
  • Minimum of five years in leadership or management experience.
  • Advanced Certified in Hospice & Palliative Nursing (ACHPN) credentialed.
  • Experience in quality improvement and program development preferred.
  • Travel Requirements: up to 20%
