American Traveler is hiring a travel RN Case Manager for a Pre-Assessment Testing (PEEC) department role in Atlanta, GA, requiring active RN licensure and Epic experience.

Job Details

• Regional office-based setting in Atlanta, GA,

• Pre-Assessment Testing (PEEC) department focused on chronic and complex condition management,

• Day shift: 8:00 AM - 5:00 PM, Monday through Friday,

• 40 hours per week, 8-hour shifts,

• 8-week contract,

• Epic EMR used in this role,

• Local candidates are accepted,

Job Requirements

• Active GA or Compact RN license required - pending licensure is not accepted,

• All certifications must be active at time of consideration - pending certifications are not accepted,

• Experience as an RN Navigator or in a similar case management/care coordination role required,

• Epic EMR experience required,

• Last 4 digits of SSN and date of birth required for consideration,

Additional Information

• Guide patients through their care journey by explaining diagnoses, treatment options, and available resources,

• Perform patient assessments, triage, symptom management, data analysis, care planning, and evaluation for patients with chronic or complex conditions,

• Serve as the primary liaison among patients, providers, families, and the broader care team to ensure coordinated, seamless care,

• Per Kaiser Permanente's HR Tenure Policy, contingent workers may not exceed two years on assignment without taking a 90-day break, regardless of whether the work was continuous or involved multiple assignments with lapses of less than three months,
