Palliative Care Nurse Case Manager

The Palliative/Geriatric Care Nurse Case Manager will be a member of the interdisciplinary palliative care team, responsible for assessing and educating members with serious and life-threatening diseases that are referred to the Palliative Care program. Under the direction of the Palliative Care Physician, the Nurse Case Manager serves as a liaison for patients, families of patients, care givers, and referring physicians.

This position plays a pivotal role in the provision of palliative care consultations, as well as providing assistance in the primary care of seriously ill patients and their families. This person will need recent clinical experience since they may also need to perform basic clinical nursing duties, i.e., starting iv’s, blood pressure checks, escorting patients, preparing patients for the physician exam, and giving injections.

The work for this role can cover the ambulatory setting and inpatient palliative care team. Candidate will perform duties within the scope of practice allowed by the State of Georgia.
