Case Manager Registered Nurse

Overview

Description

Responsible for coordinating care for identified members with complex medical conditions in collaboration with hospital physicians, ORM staff (IPCC, CM, SW, PTSP), practitioners, medical office staff and other providers. The goal is to support and facilitate a smooth transition from the acute setting or skilled nursing facility to alternative levels of care or home.

Collaborates with physicians, telephonic care coordinators, inpatient case management/social workers to create a safe discharge plan for identified complex patients.

• Assessment of identified members
• Development of a safe discharge plan from acute inpatient, skilled nursing, LTAC and Inpatient Rehab facilities
• Coordinates post-acute services and follow-up medical care to ensure continuity of care

• RN State of GA or Compact
• BSN
• BLS Certification
• 2 years of CM, UR, Discharge planning
• 5 years in a hospital setting or acute care setting
• Complex Case Management Certification preferred

• 5 yrs experience Case Management / UM
• Complex Case Management exp and/or cert
• Experience in Chronic conditions; diabetes, Cardio conditions, behavioral health

Job Type & Location

This is a Contract position based out of Atlanta, GA.

The pay range for this position is $45.00 – $60.00/hr.

This is a fully onsite position in Atlanta, GA.
