Overview
The Care Manager is responsible for care coordination, progression of care, and proactive discharge planning. The manager is accountable for expediting the timely and safe discharge for all patients in their case load.
Responsibilities- Coordinate care and progression of care for patients.
- Lead proactive discharge planning.
- Ensure timely and safe discharge for all patients in case load.
- Graduate of Nursing Program.
- Two (2) years of experience in an acute or post-acute setting.
- Current unrestricted registered nurse (RN) license in the state of Georgia.
- Certification in Case or Care Management.
- Experience using Inter Qual or Milliman.
- Master’s degree preferred.
