Position: Registered Nurse (RN) – In-Home Assessments & Supervisory Visits

Caring Hearts of Georgia Health Services is actively hiring a Registered Nurse (RN) to complete in-home nursing assessments, supervisory visits, and provide clinical oversight for services delivered to seniors and individuals needing personal support. Key Responsibilities
  • Complete initial in‑home nursing assessments and routine reassessments
  • Conduct supervisory visits to monitor the quality of care provided
  • Provide clinical oversight and guidance to caregivers (PCA, CNA)
  • Update and revise care plans based on client needs and condition changes

    Ensure compliance with agency policies, state regulations, and care standards
  • Document assessments, supervisory visits, and care plan updates accurately
  • Communicate effectively with clients, families, and staff
  • Support safe, appropriate delivery of all services
Qualifications / Requirements
