Job specification

• Oversee and coordinate all aspects of patient care within the home health program
• Conduct comprehensive patient assessments and develop individualized care plans
• Supervise, mentor, and support nursing and clinical staff to ensure quality outcomes
• Collaborate with physicians, therapists, and other healthcare professionals to deliver coordinated care
• Monitor patient progress, adjust care plans as needed, and ensure compliance with regulatory standards
• Review clinical documentation for accuracy and compliance with agency and state guidelines
• Provide direct patient care as required, ensuring compassionate and evidence-based nursing practices
• Participate in performance improvement initiatives and contribute to policy development for home health services

Candidate requirements

• Current Georgia RN license (or eligibility for licensure)
• Bachelor of Science in Nursing (BSN) required;
  Master's degree preferred
• Minimum 2-3 years of nursing experience in home health, hospice, or community health
• Previous leadership, charge nurse, or case management experience preferred
• BLS certification required
• Strong knowledge of Medicare, Medicaid, and home health regulations
• Excellent communication, leadership, and organizational skills
