Overview

Clinical Patient Services Coordinator/Scheduler LPN – Job Description

Responsibilities
  • Clinical Patient Care Coordination:
    Coordinate patient care activities, including scheduling appointments, managing patient records, and ensuring care practices comply with established standards.
  • Communication Liaison:
    Act as a bridge between patients, families, and healthcare providers to facilitate communication for comprehensive care delivery.
  • Administrative Support:
    Assist with administrative tasks such as maintaining accurate patient records, managing documentation, and ensuring compliance with healthcare regulations.
  • Quality Improvement:
    Participate in quality improvement initiatives to enhance patient care and clinical operations within the facility.
Duties
  • Maintain inpatient charts in the approved chart form sequence while the chart is current.
  • Medical records are updated daily as new information becomes available and updates are filed in the designated area of the chart.
  • Promptly schedule both on-campus and off-campus appointments/diagnostic tests.
  • Keeps a written log of patient appointments.
  • Communicate with admissions for each off-campus appointment.
  • Communicate discharges and cancelled appointments to admissions and receptionist.
  • Obtain written prescriptions and referrals as required.
  • Answer the telephone promptly, identify self and location.
  • Receive information and take telephone messages in a pleasant, professional manner and channel important telephone calls to the appropriate individuals.
  • Maintain patient confidentiality at all times.
  • Keep stationery supplies available to the department as needed.
  • Oversee and participate in daily housekeeping of the nursing station.
  • Assist in department coverage as assigned (Treatment Plan Updates).
  • Provide clerical support including labeling files and charts, filing forms in patient charts, and general clerical duties.
  • Maintain the daily department census sheet/board.
  • Maintain order and safe storage of department forms and documents not part of the medical record.
  • All other duties as assigned.

Qualifications
  • Requirements:
  • GA license
  • Minimum 2 years experience as an LPN. Experience in an office setting and behavioral health preferred.
