As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of physicians, APCs, registered dietitians, registered nurses, and registered pharmacists to provide comprehensive medical care with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity-related conditions in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.

Schedule:

Monday–Friday with flexible scheduling, 32 patient contact hours minimum (+ 8 hours of administrative time). Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone on-call. EMR:
