You will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.
This is not a Remote position
Travel will be reimbursed at the state approved rate
Responsibilities- Provide home‑visitation services to medically diagnosed high‑risk, pregnant, post‑partum women and their infants
- Complete required documentation per the DPH Home Visiting Program guidelines to eligible women and infants
- Work with a team to provide case management services to high‑risk perinatal women and infants through home visitation
- Complete quarterly reports
- Collaborate with healthcare professionals and community partners to provide person‑centered support to individuals and families
- Participate in health fairs, educational programs, and in emergency preparedness & response activities
- Candidate must have a valid, unexpired driver’s license and reliable transportation
- Daily travel throughout West Central District’s 16 counties is required for this position. Travel will be reimbursed at the state approved rate of 0.725 per mile.
High School Diploma/GED and three (3) years of experience in a social services‑related position or one (1) year of experience at the lower‑level Social Services Tech 2 (SST
011) or position equivalent.
Home visitation experience and public health experience preferred. Certified Nurse Assistant or Medical Assistant certificate preferred. Experience with screenings, obtaining clinical data, and interviewing patients. Prenatal care experience working with high‑risk mothers and babies.
