Community Health Worker - Choice Neighborhood

Join Morehouse School of Medicine to apply for the Community Health Worker - Choice Neighborhood role. The Department of Community Health and Preventive Medicine seeks an immediate hire to support the CHOICE Neighborhood program. The CHW will build trust with residents, reduce barriers to care, and connect families to critical health and social services. This position focuses on promoting health equity and strengthening partnerships among residents, service providers, and community stakeholders.

• Education: High School Diploma or General Equivalency Diploma (G.E.D.)
• Experience: A minimum of two (2) years of related experience in community health programs.

• Education: Associate’s degree from a regionally accredited institution.

• Build trust and serve as a link between communities and healthcare/social services.
• Provide health education and resources tailored to community needs.
• Help residents navigate healthcare systems, including scheduling, insurance, and referrals.
• Advocate for residents by identifying barriers to care (transportation, housing, food insecurity, insurance) and linking them to appropriate community resources.
• Collaborate with local partners to advance the CHOICE Neighborhood program’s health and wellness outcomes.
• Track services, document outcomes, and support program evaluation.
