Functions in the specialty role of Diabetes Educator by providing direct patient care in the acute care and ambulatory settings based on consultative requests from the medical staff. Works closely with the entire medical staff and nursing staff to prepare patients for discharge in a safe, efficient manner. Provides tools and skills for homecare to patients and families. Makes recommendations to hospital staff for changes in diabetes management.
Serves as an educational and informational resource to all hospital staff and the community.
Education
- Graduate of a Nursing program or Bachelor's of Science in applicable field.
Work Experience
- Two (2) years of experience in a clinical setting preferred.
- Experience in Diabetes Education preferred.
Licenses and Certifications
- Current License as a Registered Nurse (RN), Registered Dietitian (RD), Certified Diabetes Educator (CDE), or Diabetes Care and Education Specialist (CDCES) required
