A Day in the LifeYou’ll advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem‑solving skills, you’ll coordinate care plans with physicians and families, facilitate patient transfers, and address barriers to accessing specialized post‑ICU care.
Key Responsibilities- Work with physicians, discharge planners and patient families to plan the best continuum of care after an ICU or post‑surgical stay.
- Perform clinical assessments of referred patients; counsel families to assist them in making the best care decisions.
- Partner with hospital case managers and physicians to create a smooth pre‑admissions and transfer process, removing payer‑related and operational barriers.
- Registered Nurse (RN) or Respiratory Therapist (RT) license.
- Ability to complete clinical assessments of medically complex patients.
- Demonstrated success in patient advocacy.
- Superior communication and relationship‑building skills.
- Prior healthcare marketing or business development experience is preferred; extensive training is provided.
- Full‑time:
Monday‑Friday 8‑5pm. - Competitive salary plus uncapped monthly bonus incentives.
