As an Independent Optometrist leasing space inside Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being. You will have autonomy to set your practice days, hours, patient schedule, fee schedule, and keep 100% of exam/professional fees.

Launch your services in Sam’s Clubs. Provide eye and vision care, determine opening hours, manage patient appointments, set fees, maintain records, and ensure compliance with state regulations.

• Establish and manage practice schedule and operating hours.
• Control patient scheduling and maintain quality care.
• Set fee schedules and retain full revenue from services.
• Maintain equipment and instrumentation, where applicable.
• Provide administrative and opt‑tech support services, if allowed by state.
• Ensure compliance with all licensing and insurance requirements.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
