Responsibilities

• Evaluate patients' functional status and all relevant body systems for CMS documentation.

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient care needs related to mobility, endurance, pain, and other medical conditions. Based on this, they develop treatment plans, perform interventions to improve patient well-being, and monitor progress.

Assist physicians in developing therapy plans of care, observe and report patient responses, treat patients to relieve pain and restore function, and educate the healthcare team, patients, and families about the care plan and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Tennessee
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
