Position at NHC Home Care Athens is looking for a Medical Social Worker to join our team. The Medical Social Worker supervises and provides the evaluation of the social and emotional needs of patients related to their medical condition, and provides counseling based on this evaluation.
Qualifications – All States- Master’s Degree from a school of social work accredited by the Council on Social Work Education.
- One year social work experience in a health care setting is required.
- Current state certification as a certified master social worker (CMSW).
- Current license as Licensed Clinical Social Worker (LCSW).
- Provides and accurately documents direct social services to patients to restore optimum social and health adjustment.
- Assesses social and emotional factors to minimize impact on the patient’s health and ability to cope with problems of daily living.
- Helps patients and caregivers to understand, accept, and follow medical recommendations.
- Assists patients and caregivers with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.
- Identifies resources, such as caregivers and community agencies, to assist the patient in resuming community life, including discharge planning.
- Assists providers and home care team members in understanding the significant social and emotional factors related to health problems.
- Participates in the development of the care plan.
