Provides nursing care in the home setting related to home infusion therapies for patients ranging in age from adolescents through geriatrics. Responsible for the overall management of an assigned case load of patients, coordinating care with other nurses, multiple disciplines, physicians, and community resources to achieve improved health and quality outcomes for patients. Demonstrates professional growth and accountability.

Education

• Graduate from a nursing program Required
• Bachelors Degree Preferred

Work Experience

• Minimum of 6 months of nursing experience in acute care medical/surgical nursing Required
• 1 year Nursing Experience Preferred
• Nursing Experience in home health setting Preferred
• Prior case management experience Preferred

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Current License in the State of Georgia as a Registered Nurse or NLC/eNCL Multistate License. Upon Hire Required and
• BCLS - Basic Life Support Upon Hire Required and
• DL NUMBER - Driver License, Valid and in State Employee must comply with all applicable laws related to legally operating a vehicle. Upon Hire Required

  Chemotherapy/Biotherapy Certification prior to independent chemotherapy administration Upon Hire Required
• Pediatrics:
  Identifies, reports and provides appropriate protective measures for high risk situations including, but not limited to, abuse or neglect, failure to thrive and potential for abduction. Additional Required
