Overview
The Transfer Center RN, PRN role y s Health Care System involves inpatient bed management and collaboration with nursing units, environmental services, emergency department, surgery, cardiac catheterization lab, registration, and physicians. The nurse assigns patients to beds based on admitting diagnoses, physical and psychosocial conditions including infection status, floor specialization, level of care needed, attending physician, bed availability, and presenting needs to ensure timely, appropriate bed placement.
The nurse also tracks facility data on admissions, bed placement delays, and other quality indicators, communicates with physicians to ensure correct status orders are placed on patient records, and helps ensure accurate principal and secondary diagnoses in collaboration with the treating physician.
- Collaborate with nursing units, environmental services, ED, surgery, cardiac catheterization lab, registration, and physicians to assign patients to appropriate beds.
- Assess patients based on admitting diagnoses, physical and psychosocial conditions, infection status, floor specialization, and level of care needed.
- Monitor bed availability and patient status to ensure timely placement.
- Track data on admissions, bed placement delays, and other quality indicators; report findings as required.
- Communicate with physicians to ensure patients have correct status orders in their records.
- Assist in identifying principal and secondary diagnoses in collaboration with the treating physician.
- Current GA RN license.
- BLS required – must be obtained within initial 90 days of employment.
- Must be a graduate of an accredited school of nursing; BSN recommended.
- Must have a minimum of 2 years of nursing experience, preferably in an acute care setting.
- Excellent communication, interpersonal, and critical thinking skills.
- Indirect and periodic supervision. Must be capable of working independently.
This position requires competence in assessment, treatment, and/or care for the indicated age groups. The staff member must demonstrate knowledge and skills to provide care appropriate to the age of patients served in the assigned area, based on physical, psycho/social, educational, safety, and related criteria. The required skills may be gained through education, training, or experience.
