Position Overview Under the direction of the Director of Nursing, the LPN Nurse / Medical Records LPN is responsible for preparing medical records upon admission, maintaining records during stay, and ensuring proper closure at discharge.
This role is essential to maintaining survey readiness and documentation integrity.

Key Responsibilities Enter resident information into the electronic system for new admissionsPrepare admission records ensuring all required forms are presentReview records, MARs, and TARs for accurate transcription of physician ordersEnter physician orders into the electronic system as neededReduce records monthly and file documentation appropriatelyRetrieve records from files when requestedCompile and close discharge recordsPerform routine audits to ensure completeness, correct order, and timely signaturesEnsure physicians complete and sign discharge summaries🎓 Qualifications
Current Licensed Practical Nurse (LPN) license in good standingLong-term care experience preferredWorking knowledge of medical terminology and physician ordersStrong telephone, communication, and interpersonal skillsExcellent organizational and filing abilitiesProficient in electronic documentation systems and basic computer skillsWorking knowledge of keyboarding and data processingPatience, tact, enthusiasm, and a positive attitude toward the elderly and disabledMust be at least 18 years of ageMust be able to perform essential job functions and physical/sensory requirements🎁 Benefits Snapshot
