Position Overview
Under the direction of the Director of Nursing, the LPN Nurse / Medical Records LPN is responsible for preparing medical records upon admission, maintaining records during stay, and ensuring proper closure at discharge.
This role is essential to maintaining survey readiness and documentation integrity.
✅ Key Responsibilities
- Enter resident information into the electronic system for new admissions
- Prepare admission records ensuring all required forms are present
- Review records, MARs, and TARs for accurate transcription of physician orders
- Enter physician orders into the electronic system as needed
- Reduce records monthly and file documentation appropriately
- Retrieve records from files when requested
- Compile and close discharge records
- Perform routine audits to ensure completeness, correct order, and timely signatures
- Ensure physicians complete and sign discharge summaries
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing
- Long-term care experience preferred
- Working knowledge of medical terminology and physician orders
- Strong telephone, communication, and interpersonal skills
- Excellent organizational and filing abilities
- Proficient in electronic documentation systems and basic computer skills
- Working knowledge of keyboarding and data processing
- Patience, tact, enthusiasm, and a positive attitude toward the elderly and disabled
- Must be at least 18 years of age
- Must be able to perform essential job functions and physical/sensory requirements
