Job DescriptionThe licensed practical nurse (LPN) is responsible for assisting patients in collaboration with the provider and other members of the health care team with all clinical services available at the clinic. Clinical responsibilities include patient intake, documenting medical history, vital signs, preparing and administering injections and vaccinations, EKGs, performing CLIA waived lab testing, and performing additional medical services that fall within the scope of practice of the LPN.

- Obtains, verifies, and records health history of patients.
- Gathers pertinent health data through physical assessments and biological samples.
- Collects specimens using proper technique for laboratory analysis. Performs point‑of‑care testing for all testing done in clinic.
- Provides patient care as directed by physician or advanced practice clinician and is able to make skilled judgments based on patient presentation and protocols in place.
- Documents assessments, interventions, patient/family interactions/responses, medications dispensed/prescribed, and test results in medical record.
- Assists other health care professionals in performing exams, diagnostic procedures, treatments, and minor surgical procedures in the clinic setting.
- Ensures all patient information is in accurate locations of the patient chart for quality reporting and data extraction purposes.
- Coordinates and manages care of patients seen by provider, including following up and tracking testing, referrals.
- Stays informed of insurance requirements and guidelines for patient care. Performs prior authorizations and acts as patient advocate.
- Regularly monitors patients as appropriate to promote wellness and health maintenance including preventative and chronic care.
- Participates in the development of the plan of care with other team members and safely implements that plan.
- Promotes wellness by providing patient education materials and communicating provider advice/instructions.
- Schedules, coordinates, and communicates referrals to specialists and services ordered by the provider.
