Columbia Memorial Hospital (CMH) is seeking a Psychiatric Mental Health Nurse Practitioner to join our Primary Care team to provide integrated psychiatric services to established CMH patients. Our Primary Care clinics offer a beautiful work environment where you will be able to work with adult and youth patients with a range of mental health and substance use disorders.
Practice Details- Community-based ambulatory practice where you will provide psychiatric evaluation and medication management services
- Partner with established primary care providers to provide psychiatric services to adult and youth clients.
- Mix of new and follow-up patients.
- Medical Assistant support to coordinate client care, assist with insurance pre-authorization, and caseload management.

- Successful completion of an accredited Psychiatric-Mental Health Nurse Practitioner program.
- Psychiatric-Mental Health Nurse Practitioner certification.
- Must have or acquire an unrestricted state of Oregon license.
- Possess a current, valid, unrestricted DEA with an x-designation.
- American Heart Association BLS or ACLS certification.
- Two years of experience as a Psychiatric Nurse Practitioner, preferred.
