Job Summary
LPN – Home Health with Providence Medford Medical Center (Medford, OR).
Full‑time, day shift. The role involves providing quality nursing care to patients in their homes, assisting with assessment data collection, and implementing the plan of care within the scope of licensure. LPNs work as part of an interdisciplinary team to deliver whole‑person care and support the Providence mission and philosophy.
Josephine and Jackson counties, Oregon. Work site:
Providence Medford Medical Center, 1111 Crater Lake Ave, Medford, OR. Pay range: $29.14 – $44.57 (base only; additional compensation possible).
• Work under the direction of a registered nurse to provide quality nursing care to patients in their homes.
• Assist with collection of assessment data and provide input to care planning.
• Implement the plan of care within the scope of licensure.
• Perform all duties in a manner that promotes teamwork and reflects the Providence mission and philosophy.
• Maintain licensure and stay current with clinical skills and certifications.
- Graduate of an accredited LPN/LVN school of nursing.
- National Provider BLS – American Heart Association certification within 30 days of hire.
- Oregon Licensed Practical Nurse upon hire;
Washington LPN upon hire if practicing within that state. - 1 year experience in nursing (long‑term care, acute care, ambulatory care, or home health/hospice care).
- Must comply with state laws and requirements for driving if the role involves patient travel; provide proof of driver’s license and auto insurance.
- Criminal background check upon hire in compliance with Providence St. Joseph Health – OR.
- Experience in home health, hospice, or public health.
