Position: Rehabilitation / Sub-Acute RN (Registered Nurse)

$1.75/hr Evening Shift Differential, $1.50/hr Night Shift Differential, $1/hr Weekend Shift Differential. New grads welcome.

Rehabilitation/Sub-Acute RN

Full Time or Part Time.

Base Rate: $32/hr to $33.80/hr.

Department:
Health Care Center - Skilled Nursing.

Responsibilities
  • Maintain the standards of nursing care and implement the policies and procedures of the facility’s nursing department, including skin grids, treatment plans, preventative skin care plans, fall prevention care plans, incident reports and follow-through prevention plans.
  • Maintain current knowledge and skills with respect to facility activities, nursing procedures, and technology.
  • Attend team care conferences as needed.
  • Provide oral instructions to nursing assistants and LPNs.
  • Assess residents and record all pertinent information in resident records, both written and EMRs.
  • Provide resident care and activities in accordance with prescribed medical and nursing treatment plans.
  • Administer medications and IV therapy according to facility policies and procedures.
  • Maintain current CPR skills.
  • Function as team leader and supervisor.
  • Supply and interpret information, support decisions, and facilitate communication with residents and their families, nurse aides, and support staff.
  • Ensure that residents’ call lights and alarm sounds are answered promptly.
  • Report changes in residents’ health status to physicians, NPs, and other clinicians.
  • Receive and record oral telephone medication and treatment orders from physicians or physicians’ offices.
  • Protect and maintain all work areas in accordance with facility safety policies.
  • Ensure floors, resident rooms, common areas and service rooms are free of hazardous debris.
  • Perform daily tasks in accordance with infection control and standard precaution policies and procedures.
  • Follow policies and procedures related to body mechanics and lift systems.
  • Ensure residents’ individual care plans are being followed by nursing assistants, reviewing assignment sheets.
  • Ensure that nursing assistants complete required documentation before shift end, including EMRs, restorative care, food/intake records and all other records.
  • Maintain an adequate understanding of the QA/PI process and participate in performance improvement committees as requested.
  • Perform other duties as assigned.
Requirements
  • Current/valid RN license in Ohio.
  • Current CPR certification (or willingness to obtain upon hire).
  • Onsite presence required.
