Position Summary

Provides nursing coverage to patients/families/community partners via phone and in‑person visits, including admission to hospice services as needed.

Essential Duties & Responsibilities
  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation.
  • Serve as first responder for symptom‑based calls from current patients after normal office hours.
  • Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly.
  • Participate in implementation and coordination of patient’s hospice care.
  • Assist with emergency admissions.
  • Assess the patient’s physical and mental health including the patient’s reaction to illnesses and treatment regimens.
  • Record and report the results of the nursing assessment.
  • Plan, initiate, deliver, and evaluate appropriate nursing acts.
  • Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
  • Collaborate with other health care providers in determining the appropriate patient and family‑centered plan.
  • Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
  • Provide teaching and counseling about the patient’s health.
  • Report and record the plan for care, nursing care given, and the patient’s response to that care.
  • Supervise, teach, and evaluate those who perform or are preparing to perform nursing functions and administer nursing programs and nursing services.
  • Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.
Knowledge, Skills & Abilities
  • Must be willing and able to care for patients with infectious disease processes unless approved accommodations.
  • Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
  • Knowledge of HIPAA privacy rules and regulations.
  • Knowledge of electronic medical records systems.

Minimum Qualifications
  • Two years of clinical experience preferred.
Licensures & Certifications
  • Valid NC RN license.
  • Valid Driver’s License and proof of automobile insurance.
Summary Of

Physical Requirements
  • Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling.
  • Flexion/Extension of arms/wrist.
  • Hand dexterity to include gripping, pushing/pulling and writing.
  • Lifting/carrying of 50 lbs. often.
