MDS Nurse (RN) at River Bend Health and Rehabilitation

We are seeking an experienced MDS Nurse (RN) to join our team at River Bend Health and Rehabilitation, a premier post-acute care provider in Asheville, NC. As a vital member of our interdisciplinary team, you will play a key role in ensuring the highest quality of care for our residents.

• Conduct thorough assessments of residents' medical, social, and psychological needs, using the Minimum Data Set (MDS) system to identify and address concerns
• Develop and implement Individualized Care Plans, in collaboration with other healthcare professionals, to promote optimal resident outcomes
• Provide direct patient care, including administering medications, wound care, and other treatments as needed
• Maintain accurate and detailed records of resident assessments, care plans, and treatment progress
• Participate in interdisciplinary team meetings to discuss resident care and coordinate services
• Collaborate with families and caregivers to ensure open communication and build trust

• Current RN licensure in the state of North Carolina
• Bachelor's degree in Nursing
• Minimum of 2 years of experience in post-acute care or long-term care
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to lift, transfer, and reposition residents as needed
