Location: We are currently seeking people throughout the State of North Carolina in the following DSS Region:
- Region 1 counties: Buncombe, Cherokee, Clay, Graham, Haywood, Henderson, Madison, Polk, Swain, Transylvania, Yancey, Macon, Jackson, Mitchell.
- Region 2 counties :
Alexander, Alleghany, Ashe, Avery, Burke, Caldwell, Catawba, Cleveland, Gaston, Iredell, Lincoln, McDowell, Rutherford, Watauga, Wilkes.
Travel within your assigned DSS Region is required. When you are not in the field, you will work virtually from your home. These roles are statewide field-based and require you to interact with patients, members, or providers in person four to five days per week.
This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training.

Context The information below describes the scope of the program (CFSP) and the role within Elevance Health.
CFSP context We are partnering with North Carolina DHHS to operationalize a statewide Medicaid Plan designed for Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system to receive seamless, integrated care. Within the Children and Families Specialty Plan (CFSP), members have access to Physical health, Behavioral health, Pharmacy, I/DD services, long term supports, and integrated care management.
Role purpose The Clinical Care Advisor - Prevention and Preservation coordinates operations and workflows related to case management activities in support of specialty programs (e.g., Foster Care). Acts as coach. Performs case management telephonically and/or by home visits within licensure. Manages healthcare costs for the designated population via integrated (physical health/behavioral health) case management and whole person health. Manages the most complex cases and supports Special Programs case managers.
How you will make an impact
- Providing compassionate support to children and families involved in the CFSP Foster Care system, with a primary emphasis on strengthening family bonds and delivering services to those at risk of entering foster care.
- Conducts assessments to identify individual needs.
- Develops comprehensive care plans to address objectives and goals identified during assessment.
- Supports member access to appropriate quality and cost effective care and modifies plans as needed.
- Coordinates with internal and external resources to meet identified needs of the member in terms of integrated whole person care (physical and behavioral).
- Coordinates social determinants of health to meet the needs of the member and incorporates that into care planning.
- Works closely with various state agencies.
- Maintains knowledge of the system of care philosophy and a spectrum of effective, community-based services and supports arranged into a coordinated network.
- Builds partnerships with designated populations and their families, addressing cultural and linguistic needs.
- Evaluates health needs and identifies applicable services and resources with members and families.
- Provides patient education, medication reconciliation, and identification of community resources; assists with follow-up care.
State requirements
- Requires MS/MA in social work, counseling, or a related behavioral health field or a nursing degree with a minimum of 3 years of clinical experience in social work/counseling with complex psychiatric and substance use treatment, or an equivalent background.
- Requires an active, current license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist, as applicable by state law and scope of practice.
