Medical Social Worker

Western North Carolina Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC). We are looking for a full-time Medical Social Worker/Medical Care Coordinator to coordinate the collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes.

Base pay range: $49,000 - $53,000 per year.

Responsibilities
  • Designing health plans for patients/caregivers including using community resources to secure housing and food sustainability; connect patient with appropriate resources and support groups including potential legal and financial assistance; connect patient with substance treatment programs; connect patient with any and all resources to facilitate health and well-being.
  • Overseeing the correct implementation of the health plans including monitoring the use of community resources to ensure they are being used effectively.
  • Recording the effectiveness or reasons for failure of the health plans. Helps patient and his/her domestic caregiver address any identified barriers preventing the success of the health plan.
  • Ensure patient and his/her domestic caregivers, to the best of their abilities, understand the health plan as patient consent allows.
  • Monitors the health, psychosocial, and functional status of assigned patients for whom a health plan has been created.
  • Identify any financial or psychosocial barriers that would prevent adherence to the created health plan.
  • Accurately documents all services and outcomes achieved in a timely manner.
  • Offers emotional support to patient and his/her domestic caregivers.
  • Educates patient and his/her domestic caregiver about self-care/self-management.
  • Coordinates transportation to and from all medical and related appointments.
  • Contact patient and his/her domestic caregivers with enough frequency and intensity to verify all the tasks outlined in this Job Description are consistently carried out. (At a minimum, this requires telephone contact and making periodic visits to the patient at his/her setting (e.g. homeless shelter, home, nursing facility, etc.).)
  • Participate in mandatory in-services, drills, staff meetings and other trainings.
  • Perform duties in a prioritized, organized, and orderly manner to maximize efficiency and productivity.
  • Facilitate the maintenance of a clean, neat and well-organized work area.
  • Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.
  • Support current incentive, regulatory, and certification requirements (such as Meaningful Use, PCMH and UDS) through documentation, participation in initiatives, and other activities as directed.
  • Perform other work duties as assigned.
Qualifications
  • Bachelor’s degree in human services, social work or psychology;
    Master’s degree preferred.
  • Previous experience in care/case management.
  • Expert knowledge of medical office operations.
  • Familiarity with role of FQHCs in the community.
