Job SummaryWe are looking for a physician‑directed independent role to deliver primary care for PACE participants, including preventive, acute, and chronic disease management. The role requires experience treating complex geriatric, internal medicine, and mental health patients and no visa support will be offered.
Qualified Candidates- Board eligible/certified in any specialty
- Experience treating complex geriatric, internal medicine, and mental health patients
- Position does not offer visa support
- Schedule is M‑F 8 am to 5 pm with on‑call coverage
- Work closely with Advanced Practitioners, Nurses, Social Workers, Therapists, and Volunteers in a supportive environment dedicated to quality of life
- Provide primary care for PACE participants, including preventive, acute, and chronic disease management
- Collaborate with interdisciplinary team for care planning and oversight
